24-Hour Customized Living

 

Program Overview


Restart Inc. provides 24-Hour Customized Living housing and services for adults living with a Traumatic Brain Injury and other co-occurring disabilities. Each 24-Hour Customized Living site offers a person-centered approach to meet each individual’s unique needs.

Services at our 24-Hour Customized Living Sites include:

  • Room and Board

  • Nutritious Meals & Snacks

  • 24-Hour Emergency Response and Assistance from On-Site Staff

  • 24-Hour On-Call Nursing: RN/LPN

  • Housekeeping & Laundry Services

  • Advocacy & Social Services Support 

  • Medication Management & Healthcare Coordination

  • Home Health Services - Including Assistance with Activities of Daily Living and Assistance with Mental Health, Cognitive and Behavioral Concerns

  • Socialization and Activities  

 

Qualifications

  • 18 Years or Older

  • Diagnosis of Brain Injury, or other Disability

  • Eligible for Public Funding, or working on Eligibility

    • Services funded through a Waiver Program (CADI, BI or EW)

    • Room and Board Funded through GRH (Group Residential Housing) or paid privately (SSI, SSDI, RSDI)

  • Kathy’s Place: Located in South Minneapolis near Lake Harriet

    • 9 Bedroom Residential Home

  • Minnehaha: Located near Minnehaha Falls in South Minneapolis

    • 8 Bedroom Residential Home

  • 614: Located in the Marcy-Holmes Neighborhood in Minneapolis

    • 7 Bedroom Residential Home
